Please complete the form below to request a change of address to your air purifier application. You will also need to upload ONE of the following Proof of Residency documents:

• SDGE Account Transfer request with the applicant ‘s name, effective date, and complete address including unit number, if applicable.
• Signed Lease agreement for current address with the applicant ‘s name, effective date, and complete address including unit number, if applicable.
• USPS change of address form with the applicant ‘s name, effective date, and complete address including unit number, if applicable.
